The purpose of this form is to evaluate a player’s overall experience of her hockey season and to improve our coaching talent.

This form is made available online with all parent or players encouraged to fill it out.

 

 

The Evaluation Form is in two different formats.

 

1.      PDF: can be printed off, filled out, and mailed or passed on to an Executive Member. To view PDF click here.

2.      Word Document: this allows you to fill out the form on the computer, save it onto your computer, and email it back to the appropriate member. Before filling out the form press the Insert key on your computer keyboard. To view Word Document click here.
